Lisa Plauche, NP-C
Lisa Plauche’ has 16 years of experience as a healthcare professional. She is an AANP board-certified family nurse practitioner and received her Master of Science in Nursing from Loyola University New Orleans. She is enthusiastic about providing quality, compassionate, and comprehensive care to post-acute patients and long-term care residents. Prior to joining Continuum Care Partners, Lisa’s experience as a nurse practitioner includes transplant, stroke neurology (as both Program Coordinator and Stroke Clinical Research Coordinator), as well as in Hospital Medicine. Her broad experiences provide the base for supporting patients through a compassion and collaborative care approach that facilitates treatment and the patient’s sense of comfort and trust. She is a member of the Society for Post-acute and Long-term Care Medicine and the American Association of Nurse Practitioners. In her free time, Lisa enjoys traveling and spending time with her twins and family.
